Free and reduced price meals
This one is done in person, with us.
Meal benefits use an official West Virginia state form that needs a signature and some household details, so we complete it together at the front desk. It takes about ten minutes. This page tells you exactly what to have with you so one visit is enough.
Bring, or be ready to answer
- Everyone under your roof. The names of all children in your household, including infants and children at other schools, plus the grade for each school-age child and the total number of people in the household.
- A SNAP, TANF, or FDPIR case number, if you have one. If anyone in the household receives one of these, the case number is all the form needs. The income questions are skipped entirely.
- Otherwise, household income. For each adult: earnings from work before taxes and how often they are paid (weekly, every two weeks, twice a month, or monthly), plus any child support, alimony, public assistance, pensions, Social Security, or other income.
- Last four digits of a Social Security number. Only the last four, from the household's primary wage earner. If there is none, the form has a box for that too.
Good to know
- Foster children, and children who are homeless, migrant, or runaway, qualify automatically. Just tell us and we handle that part of the form.
- One application covers every child in your household.
- Want to read the form before you come in? The official state PDF is below.
Call us at (304) 487-1222 and we will happily walk through the checklist on the phone first.
The form itself
Read it here, sign it with us. This is the official 2026-2027 West Virginia application, so you can see every question ahead of time. It still has to be filled out and signed in person at the center. Please do not print and complete it at home, because the state requires the signature with us.
